New ESI Form
Employers under the Employees’ State Insurance Corporation scheme should submit return of contributions in a revised Form – 5 with effect from April 1, 2008.
A release from the corporation said that the revised form had a self-declaration by the employer on the employees engaged by/through principal/immediate employers and their coverage, submission of employees’ declaration forms, distribution of temporary identification certificates, permanent identity cards to insured persons and wages for payment of contribution and wages excluded for such purpose. The self-declaration should be supported by a certificate from a chartered accountant for employers with 40 or more employees. For details, log on to: http://www.esicoimbatore.org/
